The development of the HealthConnected platform began in 2017 under the aegis of the HONK Group and its then director Paul Witteman. In mid-2019, HealthConnected was corporatized to enable a national rollout. HealthConnected provides a regional healthcare IT platform, including full Electronic Patient Record (EHR) functionality, that connects primary care. Primary care providers use the platform for reporting and communication, referral and consultation, among other things. HealthConnected has a focus on healthcare regions, of which there are more than 100 in the Netherlands.

HealthConnected provides a modern web-based platform for healthcare providers, where a fully integrated patient record is available in real time and from any location. The platform is active in several healthcare regions and is expanding in the near future. With this, HealthConnected is responding to the trend of regionalization and the need for ease of use.
